Request to Exceed Credit Restrictions

This form is used to request permission to exceed credit restrictions imposed due to being on academic probation or academic restriction. Please note that the result of this request will not change your academic standing; you will remain on academic probation or restriction even if your request is approved.

IMPORTANT NOTE: Please do not use this form if you have received an academic warning as there are no credit restrictions placed on students who have received an academic warning. This form cannot be used to appeal your financial aid status. Please speak with a success coach if you have concerns about your financial aid status.

Please fill out this form completely, truthfully, and accurately. Incomplete forms will not be considered. You will get confirmation by email that your application has been received by the Academic Standards Committee within 10 business days of submitting this form.
